The Connections Difference
The Shoreline Therapy Center’s Connections Program provides intensive, comprehensive community-based behavioral health care to children, adolescents and adults. Essentially, Connections is a Community Based Integrative outpatient program. We treat clients who have been struggling for years and those that are new to exploring treatment options.
We treat clients who might otherwise be heading towards residential care placement as well as clients who are stepping down to a lower level of care from inpatient or residential settings and are transitioning back to their homes. The Connections Program fills the void in therapeutic services by providing a continuum of care. We have the ability to deliver many of the same services clients have experienced in 24-hour care settings, less restrictively, keeping clients at home and without disruption to their community-connections.

Team-Based Approach
Instinctively and through our collective experiences, everyone on our team is firmly dedicated to teamwork. It allows us to look at each client from multiple perspectives. It gives us a quality control mechanism to assure the very best form of treatment is given to each individual client. Through the inclusion of various roles representing different disciplines, our team-based approach allows us to treat our clients holistically, touching all aspects of their lives. As a team, we meet frequently to insure that the individual client’s program is proceeding on plan as well as to better understand their progress from all relevant perspectives.
The program has the ability to treat conditions across the diagnostic spectrum
Our therapeutic interventions are directive and solution-oriented. Our treatment model employs a team led by a licensed therapist that includes case management, individual and group therapy, home-based counseling, and a health and wellness program. Our clients are actively monitored by their team. Together (client, team, and family) we set goals, develop action plans, monitor progress, and adjust as needed.
Aim to Achieve functional independence
When an individual at any age or stage becomes disconnected from their communities, be it school, the workplace, their neighborhood or home, they are no longer functioning independently. They are living in partial or complete isolation, struggling to make it through each and every day. The job of the Connections team is to understand the dynamics underlying a client’s detachment from their communities and then begin to build back effective and functional attachments.
A Multi-Disciplinary Approach to Treatment
Our deeply experienced team comes from a variety of backgrounds. We are an open system that is comfortable partnering with providers. Recognizing the mind-body connection, we are happy to partner with pediatricians and family physicians to assist in treating physical illness.
Focuses on Both, the Individual and the Environment
For us, the environment includes family, friends, educational and community settings, and extra-curricular activities. We will collaborate with schools and employers and work with clients in their homes to both enhance our understanding of the client and to develop the client’s points of contact in their lives. It is in fostering and strengthening these relationships that we assist clients and their families in pursuing the skills and supports necessary to transition to emotional and functional independence.

Exclusions, Limitations and Caveats
Since we are not a residential facility, there are certain situations or cases we cannot take. However, if you are uncertain about whether we can help address your situation, do not hesitate to call us. If your situation is one we cannot treat, we would be happy to direct you to a therapist or program that can. The following are situations we cannot treat:
- Actively Aggressive Individuals: Displaying a propensity to cause harm or violence towards self, others or property.
- Active Substance Abuse/Addiction: While we work with many recreational users, we do not treat those with a serious dependence on any substance.
- Unsafe Home: We do not treat victims of abuse, violence or neglect.
- Medical Intervention: We cannot provide medical intervention or dispense prescription medication or treat wounds or injuries, nor provide ambulation or hospitalization.
